New Patients

At Doyle & Taylor Physical Therapy, we are very aware that your time is valuable. With that in mind, if you are running late for an appointment, please call as we do our best to accommodate you, but we may need to reschedule, so that you will able to have a full treatment. If you find that you are unable to keep your scheduled appointment, please provide 24 hours notice. Expect your initial appointment to be approximately 1 hour and subsequent appointments to last approximately 45 minutes to 1 hour.

 

 

 

What to Expect on Your First Visit

Please arrive 10-15 minutes early for your first visit. Please have a referral from your physician, dated within the last 30 days, if required by your insurance. Our receptionist will ask you to complete the appropriate forms so that we can submit claims to your insurance carrier. You may do most of this paperwork in advance.  Once you have scheduled your initial appointment we can send you any necessary paperwork. You will also need to have your driver's license or photo ID as well as your health insurance cards. We will call to verify your insurance before your first visit; however, we strongly urge you to understand your benefit prior to starting physical or occupational therapy by calling the number on the back of your insurance card.

Your therapist will then do a thorough evaluation and develop an individualized treatment plan. Treatment will usually begin on your first visit.

 

What to Wear

Please wear comfortable clothes. Your therapy sessions could involve exercising so you may be asked to bring shorts, a T-shirt and appropriate footwear. Gowns will be provided as necessary.

Doyle & Taylor Physical Therapy participates with most major health insurances, including Commercial plans, Medicare, and many Workers' Compensations. We will also bill automobile insurances for your services if you have been in a car accident.  We do not hold for settlement.

Doyle & Taylor Physical Therapy will file your insurance claims for you.

 

Most insurance policies do provide benefits for outpatient therapy services; however, these benefits are different for each policy. Doyle & Taylor Physical Therapy will verify your insurance benefits; however, it is ultimately your responsibility to obtain this information. We highly recommend that you contact your insurance carrier and learn what your benefits are , what your insurance policy may require you to pay such as a deductible, a co-pay, and/or a percentage of the costs of your therapy treatment, as well as if you are required to have a Primary Care Physician (PCP) referral. There should be toll free number on your insurance card that you can call to obtain this information.

 

Patient's responsibilities include:
  • Current medical prescription for PT or OT if required by your insurance.  Medicare requires a referral to be dated within the last 30 days, NO exceptions.

  • Payments of Deductible

  • Co-insurance or co-payments per visit

Payment Options:
  • Payment is due at the time service is provided.

  • We accept Visa, MasterCard, cash and personal checks.

  • If you are not using insurance: First visit - $150.00 for the initial consultation, evaluation, assessment and treatment by a licensed physical therapist. Charges for each visit thereafter will be billed based on the amount and type of service rendered.

Additional Fees: 
  • There is an additional $20 dollar fee for completion of forms for use outside of the patient's medical record, including:

    • Day Care

    • Pre-Employment

    • Employment

    • Life Insurance.

    • Family Medical Leave
