Doyle & Taylor Physical Therapy will verify your insurance benefits; however, it is ultimately your responsibility to obtain this information. We highly recommend that you contact your insurance carrier and learn what your benefits are , what your insurance policy may require you to pay such as a deductible, a co-pay, and/ or a percentage of the costs of your therapy treatment, as well as if you are required to have a Primary Care Physician (PCP) referral. There should be an "800" number on your insurance card that you can call to obtain this information.
Patient's responsibilities include:
Current medical prescription for PT if required by your insurance. Medicare requires a referral to be signed and dated within the last 30 days, NO exceptions.
